Who we are

This website is operated by LifeCare Chiropractic, a chiropractic clinic at 1830 S Alma School Rd, Ste 135, Mesa, AZ 85210. You can reach the office at (480) 839-2273 or contactus@lifecarechiro.com.

This site is a public marketing website. It is not a patient portal, online chart, or a place to send clinical records. Browsing the site does not enroll you as a patient.

When this policy applies

This policy is effective August 28, 2026. It covers information collected through this website and ordinary emails you send us from it. It does not replace the privacy practices that apply to clinical records created in the office.

Information we collect

The contact form on Contact & Schedule does not post to our servers. Choosing “Request an appointment” opens your own email app with a draft addressed to contactus@lifecarechiro.com. We receive what you send only if you actually send that message. The draft is built from these fields:

• Name (required)
• Email address (required)
• Phone number (required)
• Optional “I’m looking for” checkboxes (for example, chiropractic adjustment or a listed condition or service)
• Optional message

If you email or call us directly, we also receive whatever you choose to include in that conversation.

Our hosting provider (Vercel) keeps ordinary website access logs. Those logs can include your IP address, browser and device details, the pages you request, the referring page, and the time of the request. We do not run a separate first-party analytics product on this site.

Your browser may also store a local preference under the key lc-cookie-consent after you acknowledge the cookie notice. That preference stays on your device. See our Cookie Policy.

How we use information

We use website and email contact information to:

• Reply to questions and appointment requests
• Call or email you back at the number or address you provided
• Operate, secure, and fix this website
• Understand basic technical problems (for example, a page that will not load)
• Meet legal obligations that apply to a small clinic website

We do not use this website to run ads based on your browsing, and we do not build a marketing profile from the contact form. A submitted request is not a confirmed visit until the office replies.

We do not sell your information

We do not sell, rent, or trade personal information collected through this website. We do not share it for cross-context behavioral advertising.

Service providers who help run the site

We use other companies to host the site and to load media that appears on some pages. They process technical data as needed to provide those services:

• Vercel hosts and delivers the site. Request logs and related infrastructure data are processed in the course of hosting.
• Your email application and email provider handle the draft created by the contact form. We do not operate a web inbox on this site.
• Fonts — heading and body type (Bitter and Montserrat) are loaded through Next.js font files served with this site. Material Symbols icons are loaded from Google Fonts.
• Bunny Stream (mediadelivery.net and Bunny’s video CDN) serves the homepage hero video and the video player.
• Google Maps provides the office map embed and map link on the contact page. Interacting with the map may send data to Google under Google’s own terms.

Those companies act as independent services for their own products. We ask them only to do the job described above. We do not control cookies or logs they set on their own domains.

Please do not send detailed medical information here

This website is not designed for protected health information. Do not use the contact form, a website email, or a public message to send exam findings, imaging, lab results, medication lists, insurance cards, Social Security numbers, or a full health history.

A short note that you would like to be seen — for example, “low back pain, afternoons work better” — is enough for us to call you back. Save clinical detail for a phone call or an in-office visit.

Clinical records and HIPAA

Patient charts, billing records, and other clinical information are created and kept in the office under the privacy rules that apply to health care, including HIPAA. This website is not a HIPAA patient portal, and sending a marketing-site email does not open an electronic medical record.

If you become a patient, the office will explain how your clinical information is used and how to request records through the clinic — not through a login on this site.

How long we keep information

Emails and phone messages we receive are kept as long as we need them to respond, to schedule, and for ordinary business or legal reasons. We do not publish a fixed deletion date for every message.

Hosting logs are kept according to Vercel’s practices and our need to operate the site. The lc-cookie-consent preference remains in your browser until you clear site data.

Clinical records, if you are a patient, follow the clinic’s medical-record retention rules — not this website policy.

Security

We use reasonable administrative and technical steps appropriate for a public clinic website (including hosting on a commercial platform and limiting what the site itself collects). No method of transmission or storage is guaranteed to be secure, and we do not claim a specific security certification for this site.

Because the contact form opens your email app, the privacy and security of that message also depend on your device and your email provider.

Children

This website is written for adults considering care for themselves or their family. It is not directed at children under 13, and we do not knowingly collect personal information from children through the site. If you believe a child sent us information here, contact us and we will delete it from our website inbox.

Your choices and access

You can:

• Decide not to use the contact form, or omit optional fields
• Call the office instead of emailing
• Ask us to correct or delete contact information we hold from a website email
• Control cookies and site data in your browser, as described in the Cookie Policy

To review or update a message you sent through the site, email contactus@lifecarechiro.com or call (480) 839-2273. Requests about a patient chart are handled in the office, not through this page.

Who this site is for

The site is intended for people in Arizona and elsewhere in the United States who want information about LifeCare Chiropractic. We do not target other countries, and we do not represent that this policy meets every non-U.S. privacy statute.

Changes

We may update this policy when the website or our practices change. The effective date in this policy will change when we do. Continued use of the site after an update means the new policy applies to later visits.
