Iron deficiency results in decreased oxygen delivery to tissues. Decreased oxygen leads to increased oxidative stress (seems a little paradoxical, huh?) which is a player in most chronic diseases. So it should come as no surprise that anemia was a risk factor for mortality. Also, since iron, in unbound forms, is a pro-oxidant, too much also leads to the same problem.
