***Noninvasive diagnosis of ruptured peripheral atherosclerotic lesions and myocardial infarction by antibody profiling***
Many patients have their routine stress test and come away with a sense of relief that nothing was found. Unfortunately, these patients can be just as likely to drop dead of a heart attack leaving the center because of the concept of unstable plagues. We are all aware of the concept of atherosclerosis and plaquing in the arteries of the heart. However, what many are not aware of is the idea that a plaque in a big artery taking up 25% of the opening can break free, travel to a smaller vessel and take up 100% of that opening, leading to a heart attack. So the important concept is not just the presence of a plaque, but how STABLE that plaque is. There are lifestyle factors that play a role in the stability of a plaque (higher intakes of omega 3 fatty acids, less inflammation, etc...) but it is still difficult to asses. Certain ultrasound procedures can determine the stability of a plaque, but are somewhat invasive. This article looks at a fantastic approach with bloodwork to help asses a patient's likelihood of having an unstable plaque. Keep your eyes on this approach in the future--it'll be highly, highly valuable.

***Childhood Respiratory Disease and the Risk of Anxiety Disorder and Major Depression in Adulthood***
There are a variety of ways to look at this study, but there is no doubt that having almost 20 times the risk of developing anxiety (in infants with respiratory disease at age 1 and parent reported respiratory disease at age 7) that something is happening here. I have several possibilities. First, it is likely that there would be antibiotics at play here disrupting normal flora. We have seen several studies linking altered GI flora and increased physiological stress. Second, the factors setting up the altered respiratory responsiveness leading to respiratory disease can involve essential immune shifts. This pathological shifting of the immune system can wreak havoc in a variety of ways, including neurological. Regardless of the linking causes, these findings further underline the importance of avoiding respiratory disease in the first place (healthy pregnancy, strong bacterial flora, unprocessed diet, breastfeeding) as well as an aggressive approach to re-balance the immune system instead of mere medication management.

I've been a strong opponent of the way we treat migraines in this healthcare system ("here--take this..."). Migraine is unquestionably a cerebral manifestation of a systemic problem. If all we do is medicate the headache, we just signed an early death certificate for the patient. This is strongly confirmed by this current study linking migraine with aura and CVD--most notably in women without established risk factors. And yet, despite all the research, I have yet to EVER have a patient come in with migraines that were given any type of lifestyle advice...

***Benefit of Adherence With Bisphosphonates Depends on Age and Fracture Type***
I've always felt that anti-resorptive therapy is the complete wrong way to approach improving bone health. A few things with this study. First, take 10 minutes or so to read through the rather extensive list of conflict of interests!! Next, look at how low the adherence is. Lastly, even with high adherence to medication use, the number of patients that need to be treated to prevent 1 hip fracture is 107. This number would need to be much, much higher at lower rates of adherence found across the board in the people taking this drug. This is a very high number, suggesting that this class of drugs should not even be used, which is of course contrary to the huge number of scripts written for this class. I've always said that if the general public truly understood how useless and dangerous many of these drugs were there would be a paltry market for them.

A seeding trial is a trial done under the guise of research, but is actually created and administered by the marketing department. This particular study was run and published purely to get doctors used to writing prescriptions for Vioxx when it was first released. Really. I've said it before that if the public truly knew how they were manipulated by drug companies with drugs with marginal effectiveness and frequent side effects for conditions THEY DON'T EVEN FIX--Chiros would be on the top of the health care pile. This also makes you wonder, if the physicians in these studies are so easily manipulated, are they truly acting as "physicians" or more like technicians being trained to give drug A for condition A?

Regular readers of Updates will confirm how much I rant and rave about the critical importance of normal, healthy bacterial flora in our newborns, infants and toddlers. Words alone are inadequate at explaining just how important this is to healthy, balanced development of our immune system and gut ecology. Because of this, importance of the pregnant mom's vaginal flora (has she ever been on antibiotics?), the route of delivery (C-section or vaginal) and has the child ever received antibiotics without replacing the normal flora w/ probiotics? This study again confirms that birth by C-section alters the normal flora that is supposed to be present.

As if, given the information above, the use of antibiotics wreaks havoc on a developing infants immune system, data suggests that the rate of immediate (vs. delayed w/ destruction of normal flora) side effects from antibiotic prescription requiring an ER visit are much higher than anticipated overall, and that this high rate was even higher among infants 12 months and younger.

First of all, before you look at these numbers you have to understand that "healthy lifestyle," in this study, is really very non-specific and generally nothing near the level of what I would consider a truly healthy lifestyle. That being said, we still saw a massive risk reduction for stroke of 79% for women and 69% for men. This is practically gone when you look at the fact that these were "soft" lifestyle factors used. Despite this, the idea that lifestyle is the singlemost potent weapon against stroke remains a big secret to physicians and the general public.
