***Atypical Fractures of the Femoral Diaphysis in Postmenopausal Women Taking Alendronate***
Given that bone is a dynamic tissue, constantly re-typical areasmodeling based upon demands placed upon it, the idea of locking up this turnover with osteoclast inhibitors like alendronate (Fosamax) has never sounded like a good idea to me. While we've seen some reduction in spinal and hip fractures w/ the use of anti-resorptive agents, no one has actually looked elsewhere at what this class of drugs may do to bonein other regions of the body. Sure enough, when we look, we find increased risk of fractures in other, non-typical locations. The bottom line is that the only true way to prevent fractures is a comprehensive, lifestyle and fall prevention approach.

***Mechanism of Action of Niacin ***
While I have been aware of niacin's beneficial aspects on lipids for quite some time, I personally have not recommended niacin to patients, mainly because I've viewed it from a pharmaceutical perspective, mistakenly believing that it will lower lipid levels at a superficial level while leaving underlying physiological abnormalities untouched. Well...looks like we can all be wrong sometimes. This article suggests that niacin actually has cellular effects that shift serum lipids from to a less atherogenic profile (i.e. from a low density fatty molecule to a more dense protein molecule). So, while lifestyle changes still need to be implemented, niacin may well prove to be a powerful ally in getting lipid levels under control and protecting our vasculature in the process.

Ok. I've taken several biochemistry and chemistry classes. Water is H2O which contains hydrogen, right? So, whenever I've come across those hydrogen rich water thingees I've always considered them more hype then anything. So along comes a solid study on "hydrogen-rich" water, showing some pretty significant positive effects on diabetic markers. In my defense, I have a tendency to hold judgement on seemingly questionable therapies, falling back on the idea that we really know so little about physiology, always willing to keep an open mind to articles such as this.

You've all heard me rant and rave about how frustrating it is to see dairy on every single public health recommendation this company has every produced. There are times that I feel like the crazy guy claiming the sky is falling when I talk about how dangerous dairy is. It certainly is not the health drink that we've all been brainwashed into thinking it is. Here is another nail in the never-to-be-seen coffin of dairy. This very large study of 43,000 Japanese men found a 63% increase in the risk of prostate cancer with higher levels of dairy intake. Does this sound like a healthy addition to your child's lunch?????

***Increased exposure to statins in patients developing chronic muscle diseases: a 2-year retrospective study***
While we're on the topic of well accepted by the public but poorly represented in the literature we need to bring up the statin class of drugs. Despite the fact that every (I'm generalizing here...) primary care doctor in this country feels that "prevention" of heart disease means using a statin drug, the research on its efficacy for primary prevention of CVD is almost nil. As if this wasn't bad enough, the research on the side effects slowly continues to mount. This study looks at patients who have been diagnosed with chronic muscle disease (polymyositis, dermatomyositis, etc..) and found that these patients had a much, much higher chance of having had exposure to statins (2.73 times the risk), and this risk went up even higher when these patients also had used proton pump inhibitors for control of stomach acid production.

Regular readers of Updates know that I am strongly against the blanket destruction of H. pylori every time a patient has some type of gastric problem. I firmly believe that the presence, or rather, the pathological behavior of H. pylori is a result of the environment it is in, rather then the bacteria itself. This article suggests that the presence of H. pylori goes about as far back in time as we could envision, being present at least in our Paleolithic ancestors. Knowing this, does it sound like this might be a bacteria that is independently responsible for many gastric related disorders and needs to be eradicated on sight? Forget the fact that the course of antibiotics to destroy the H. pylori will also wipe out other protective bacteria in our alimentary tract!!

***Glycaemic control among patients with type 2 diabetes mellitus in seven European countries***
The authors conclude that the progressive decline in glycemic control in most diabetics over time despite increasing medication usage indicate "the progressive nature of the disease and need for more effective disease management/therapeutic alternatives." Maybe we should look at it from another perspective....maybe if we gave diabetics REAL lifestyle advice (lower glycemic, high plant based, no artificial sweeteners, impactful exercise, stress management, etc...) instead of relying on more and more medication, that we would not see the "progressive nature" of diabetes. This review pretty much locks up the idea that lifestyle changes are the ONLY way to manage diabetes, and that reliance upon medication for management is a slowly progressive decline into worsening glycemic control.

***Use of Thiazolidinediones and Fracture Risk ***
Continuing on the rant against medication usage in diabetes management, the TZD class of drugs has been shown to increase weight and actually increase the risk of cardiovascular events (70% of diabetics die of CVD-related causes) in patients using them. To top off this wonderful list of side effects, now we're seeing an over double the risk of fractures in patients who have used these drugs over 12 months. Again and again, we see that lifestyle changes really, truly are the ONLY answer.
