***Monitoring Osteoporosis Therapy: Bone Mineral Density, Bone Turnover Markers, or Both?***
I'm gearing up towards a series of community education presentations on osteoporosis, and I know that I'm going to catch some flak on this one. This is a topic that covers many of the fallacies we live with. Our culture is so ingrained on the idea that "milk does a body good" that some get hostile when told this is very weakly supported in the medical research. Another issue is the use of urine tests to tell how quickly we're losing bone. Despite it's availability for probably more than 10 years now, it is rarely used and most physicians are unaware of this type of testing. This is, of course, despite the fact that most drugs for osteoporosis are evaluated with urinary markers. On a final note, I will guess that in 20 years we will be doing little bone density and mostly moved to qualitative ultrasound to evaluate bone quality, not just density.

***Early Effects on Endothelial Function of Atorvastatin 40 mg Twice Daily and Its Withdrawal***
So, giving high dosages of this statin drug was beneficial for only a subset of patients, and when they were taken off the drug, the unhealthy response of their blood vessels returned within 3 days. The irony is that this is under "preventative cardiology" section in this journal. Should be under the "studies to support long term pharmaceutical use to boost profits" section. Mainstream medicine still views "prevention" as the use of drugs to impact a condiiton before a more serious event occurs. So long as they maintain this viewpoint, they will never understand wellness and what it takes to help a patient get truly healthy.

***Etiology of Insulin Resistance***
While insulin resistance is probably one of my more often used soapboxes, my heart truly lies with mitochondrial dysfunction as undoubtably this trigger that begins all chronic diseases of today's time. That is why the approach to managing many chronic diseases is so similiar--they all focus around reducing damage to the mitochondria while at the same time giving these same structures all the nutrients they need to function optimally. With the recent study published on the mitochondrial effects of calorie restriction w/o nutrient restriction, this vaults this type of an approach to the top of the list of beneficial approaches for managing all chronic diseases.

***Relationship between Helicobacter pylori infection and Alzheimer disease***
While I don't think the relationship is causal, I thought I'd put this out there anyway. Long term readers of Updates will know that I view H. pylori infection as opportunistic rather than pathogenic. Becuse of this, it is likely that those lifestyle factors that make an ideal environment for growth of H. pylori will also increase risk of AD.

***Glucose Metabolism in Patients with Essential Hypertension***
Wow!! A full 2/3 of patients in this study with high blood pressure had sugar handling problems. And I don't EVER recall a patient coming in on blood pressure meds that was told about the link with insulin resistance and the increased risk for diabetes. Talk about missing the boat on treatment!! Of course, this study was strongly supported by the pharmaceutical companies--what a great idea for cross marketing!! Find a patient with one problem that is on drugs for that problem, and link this to another condiiton that needs drugs! What more could you ask for?

***Glucocorticoids and Relapse of Major Depression***
This topic is always a tough one. Most therapists never view the physiological contribution to psychological disorders, but the evidence is strongly in support of a link. This article finds that, in those patients with an exaggerated CRH stimulation test producing too much cortisol, the likelihood of a relapse was higher. This wold indicate that the patient who relapsed had adrenal glands that were not working properly, and were improperly producting too much cortisol rather than a balance of DHEA, aldosterone and cortisol.

***Selective serotonin reuptake inhibitors and adverse pregnancy outcomes***
Some of the SSRIs have been promoted as safe during pregnancy, and yet I have major concerns with the damage to the developing fetus. Anytime a fetus is exposed to high levels of something (such as cortisol from a PTSD mother, glucose from gestational diabetes or, in this case, much higher levels of serotonin) the chance for a bad outcome after birth, when those high levels are no longer present, is there. From this study, we see the risk to the fetus is actually quite serious. I would also recommend avoiding this class of drugs during breastfeeding as well for the same reasons.

***Early Downward Trends in Neurodevelopmental Disorders Following Removal of Thimerosal-Containing Vaccines***
This is a well-referenced article that looks into the issue that has raised some serious questions about our mindless devotion to vaccinations without question. I personally believe that autism is a multi factorial disorder, but I am fairly positive that mercury in vaccines is a definate player in the picture. At what point did anyone decide that mercury, a potent neurotoxin, would be okay to be put into a vaccine that would be given to an infant? And how can anyone (including the Institute of Medicine) say that this is not a problem?

***Soy protein intake by active young adult men raises plasma antioxidant capacity without altering plasma testosterone***
I remember an article in one of my trade journals a few years back. The author of an article on soy was blasting soy as harmful. I wrote a letter to the editor noting that this attitude flies in the face of much current literature. The editor allowed her to rebut. She (a registered dietician) gave about 7 or 8 articles to support her position. After tracking down these references, I almost had to laugh. Most were in support of soy being good for us!! She was obviously just regurgitating what someone else had told her and had little original thought. This is yet another article supporting the healthy use of soy. Now, that being said, I am against the trend to "soy" everything. I truly do not feel that Mother Nature intended for soy to be mutated into a tofurkey to be eating day in and day out and taken in supplements to top it all off.

***The Changing Face of Teenage Drug Abuse The Trend toward Prescription Drugs***
We feed our kids drugs (both prescription and OTC) like they're candy. Many kids are on long term drug regimines for conditions like ADD and ADHS. Their parents both condone the children's drug regimine and take drugs themselves when needed. Why should anyone be surprised to then find that these kids will choose wisely when they get older when it comes to both illicit and prescription drugs?
