This is the kind of poorly thought out study that ends up confusing patients. This study found no benefit in 6 months' use of probiotics at 200 million CFU per day. Consider that studies in infants on effecting rotavirus use 2-3 BILLION per day, the idea of using 200 million and not seeing a significant effect would be no big shocker. The problem is that physicians who do not know any better will look to studies like this and say, "See-I told you this stuff isn't effective."

When I recommend probiotics for patients, there are two things that I mention. First, one of the problems contributing to the need for probiotics is not just the destruction by normal flora, but also the lack of good replacement sources. A 100,000 yrs ago we used to bury our foods in the soil, allowing soil dwelling bacteria like Lactobacillus to completely invade the food. Personally, I use a fridge--but it does lead to a lack of replacement source of good bacterial flora. Second, once a good balance of healthy flora is set up, good lifestyle habits have to be in place to support their growth. This article finds that diet can have a positive effect on the balance of bacteria in our gut.

While Dr. Ornish may disagree (and actually-his program was much, much more than just a low-cholesterol diet), I've never been all that concerned with dietary cholesterol. It's not as simple as more dietary cholesterol = higher blood cholesterol. In many cases, it's endogenous production driving by high insulin levels that's more imp't. But physicians still demonize high cholesterol foods that are good for us. Avocados. Eggs (especially the high omega-3 type). This study supports the fact that dietary cholesterol is not as closely tied to cardiovascular risk as many would think.

Wow. Given the high prevalence (upwards of 2-3% in some studies) this article is awesome in describing the presentation, symptoms, missed diagnosis and lifestyle ramifications of children w/ celiac disease. An article that must be reviewed by anyone who deals w/ patients or family members with this condition.

I've continued to say that mitochondrial dysfunction is the root of all evil, and have yet to see any studies that convince me otherwise. This article finds that mitochondrial function is reduced by 60% IN THE CHILDREN of patients with insulin resistance. If this just isn't the scariest thing that we can envision for successive generations, then I'd be hiding under the desk if something contained worse news. The children of insulin resistant patients are 60% less efficient at using energy, resulting in fat accumulation in their cells. If we do not get on the bandwagon against insulin resistance soon, several generations of our children will be paying the price in chronic diseases.

Isn't this just the biggest kick in the gluts? A new class of drugs, the selective COX-2 inhibitors like Vioxx and Celebrex are born and a market created by clever marketing campaigns to let people know that the "other" anti inflammatories can damage the GI tract. These drugs get handed out like candy, nearly bankrupting heathcare in some countries due to their high cost. Soon the evil side is seen in an increased tendency to clot the blood, leaving tens of thousands dead. Here's the kicker---it may turn out that this class of drugs is not so good at doing what it was supposed to do better!! Run that by the widows and orphans whose loved ones suffered untimely deaths.

I recall a study several years back that dealt with the oldest old and anti-thyroidal antibodies. Now, everyone has some degree of autoimmunity to our own tissues, and this reaction increases as we get older. In that particular study, the oldest old has less autoimmunity to their own thyroid. This study again points to the importance of healthy thyroid function in maintaining long, healthy life. So the question arises, when someone starts to experience thyroid dysfunction in their 20s, and they are put on thyroid hormone without any type of evaluation into WHY they are having thyroid problems, are we depriving them of quality of life in their later years?

I have always said that no intervention for any chronic disease will be as effective without the addition of exercise to the therapeutic approach. This article adds additional weight to this recommendation by finding that giving alpha lipoic acid along with exercise leads to a synergistic effect on improving insulin resistance.

***Early melatonin supplementation alleviates oxidative stress in a transgenic mouse model of Alzheimer's disease***
Yet another wonderful article delineating the importance of the role of oxidative stress in the onset of Alzheimer's. This mouse study found melatonin to be very protective, but mainly when giving early. The problem with that approach in humans is that most clinicians have not embraced the oxidative stress/mitochondrial dysfunction model of AD and probably won't for several decades. That means that, despite knowing that melatonin may help prevent AD in the children of AD patients, they won't get the news until it is too late. Let's just hope the info gets out in time for THEIR children to protect themselves.

Just a short gripe and then I'll go...how can something as strongly predictive as the metabolic syndrome on a large number of chronic diseases continue to go ignored by clinicians interacting with patients every day? I have no rational explanation for the disconnect, and quite frankly, there is no excuse not to educate patients on managing this condition.
